Let’s talk briefly about what’s happening in osteoarthritis. Let’s use the knee joint as an example. In the healthy knee joint you’ve got the subchondral bone, which is the bone that the cartilage is sitting on and attached to. On the bone sits the articular cartilage. On top of the articular cartilage you have the meniscus. Above the meniscus is the synovium, which is important. The synovium is the physical space in the joint, the joint cavity itself. The synovium is filled with synovial fluid. Osteoarthritis is when the articular cartilage breaks down and the space in the joint is no longer there. However, the way osteoarthritis actually starts is with a dysfunction in the subchondral bone. There are osteoclast cells in the subchondral bone. Osteoclastic cells are the bone breakdown cells. In osteoarthritis they start to proliferate. In fact, an increase in inflammation can take the bone building cells, osteoblastic cells, and turn them in to osteoclastic cells. So they initiate bone degradation. The other thing that occurs as a result of the bone breakdown is the microvasculature breakdown. The circulation that the subchondral bone has decreases. Micro vessels start to collapse and fall apart because of the inflammation. The integrity of the micro vessels start to fall apart. So you imagine all these tiny little micro vessels are starting to collapse and fall apart, which means that they’re not getting blood supply adequately. They’re not holding and transferring blood supply the way they should be. The problem is that the articular cartilage and this whole synovial space doesn’t have its own blood supply. But if your if your subchondral bone is starting to dismantle and you have, microvasculature that are starting to fall apart and dismantle, you’re not gonna get adequate circulation in that synovial space, in that synovium, that fluid that bathes the the cartilage. And so what you start to get is an accumulation of the pro inflammatory mediators that have leaked into that space, and those pro inflammatory mediators remain in that space and don’t get flushed out. And when you’re bathing the cartilage in inflammation, the chondrocytes, which are the cells that build the cartilage and put the cartilage on, those chondrocytes shift from being a cell that is anabolic, meaning it is building and repairing cartilage, into a cell that’s catabolic, which is actively degrading the cartilage. MMPs and ADMTs are the enzymes that break down cartilage. Those are being produced by the chondrocytes, the same cells that are supposed to be building and maintaining your cartilage and has been for the most of your life. Now because of the presence of that inflammation, they have switched and now are actively degrading the tissue. This inflammation causes osteoarthritis. Osteoarthritis is not just getting old. It is preventable and reversible. Most of the inflammation causing your joint pain is coming from you gut. I solved the joint pain problem.
